What is Social Prescribing and who are Community Navigators?

Social Prescribing is a non-medical support service and our team of Community Navigators is like having a personal guide on your journey to well-being. When you’re seeking information, emotional support, or practical assistance, our community navigators are here to help you explore a variety of community, health, and social care resources. Think of them as your friendly companions, ready to assist you in finding the support and options that best suit your needs and goals.

• Identifying and planning the support you need
• Managing your money and your benefits
• Supporting you to live safely in your own home
• Maintaining and adapting your home to your needs
• Filling in forms
• Going through an assessment process

We can offer our support services to anyone over 55 or to adults with complex needs across East Kent!

Carers Support

Our partners, Carers Support East Kent, offer information and support for Carers through the Community Navigation for Carers service. Carers aged 16 and over who are caring for an adult can access this service.
